Payment is expected at the time of service. If the patient would like to pursue reimbursement by their insurance company, paperwork can be requested at the time of treatment.
Each insurance policy is different. If you plan to file insurance, make inquiry prior to your first visit on coverage for out-of-network providers. Also, most insurance companies will want a referral. In Maryland, the referral or physical therapy can be from a physician, physician assistant, or nurse practitioner. Let your therapist know if you plan to file and how often, so you will be given the appropriate paperwork.
It is the patients’ responsibility to understand their own insurance policy for out-of-network providers, deductibles, coverage, and filing processes.
Virago Physical Therapy is not a Medicare Provider. If you are receiving Medicare benefits you are not able to pursue treatment at Virago Physical Therapy, unless you require maintenance services or have already reached your full benefit for physical therapy for the year
Referrals and Pre-certification
Some plans within CareFirst and BlueChoice require you to have a referral from your primary physician before we can see you for your first visit. The patient is responsible for knowing if a referral or authorization is required, obtaining that information and bringing it with you to your evaluation. Due to insurance requirements, patients arriving without a valid referral may be asked to reschedule their appointment. Medicare requires a referral from your Primary or referring doctor. If you have a secondary insurance along with your Medicare plan that we do not participate with, you will be responsible for the remaining 20% that will not be covered by your secondary insurance. The 20% will be based on the Medicare fee schedule and not Virago's concierge rates.
Copayments, Coinsurance, and Deductibles
Co-payments, co-insurance and deductibles and any previous balances are due at check-in. We accept cash, personal checks and credit cards. Patients who anticipate difficulty in paying for medical services are encouraged to contact us. Payment plan arrangements will be made prior to the appointment and finalized at check-in. We are available to assist you with any questions regarding your insurance company requirements or your account balance. Please call 443-333-8792 for insurance concerns.
Concierge Rates
Most insurances will provide partial reimbursement for self-pay patients. Each insurance is specific with their rates. We encourage the patient to contact their insurance with details on their specific reimbursement rates. Virago PT will provide the patient with a bill after each visit to submit for partial reimbursement.

Fees
Thank you for your interest in Virago Physical Therapy, for specialized physical therapy in Women’s Health with emphasis on wellness and improved function. Prices are as follows:
  • Initial Virago PT Evaluation plus Treatment: $185.00 (1 hour)
  • Initial Virago PT Evaluation plus Treatment: $245.00 (90 minutes)
  • 60-minute Hour Virago PT Treatment (recommended): $150.00 (1 hour)
  • 90-minute Virago PT Treatment: $210.00 (90 minutes)
  • 45-minute Virago PT Treatment: $130.00 (45 minutes)
Your First Visit
For most conditions, home instructions will be given for starting a home program, including pain relieving techniques, posture and ergonomic instruction, and/or therapeutic exercises. These instructions are very important to the success of your therapy. You will need a doctor’s prescription for physical therapy in hand before the first treatment if you are planning to file with your insurance company. Clients who are receiving benefits from Medicare are only allowed to see a non-participating physical therapist for maintenance/wellness care which is not covered by Medicare. Clients are responsible for filing their own insurance claims, and if you are planning to, I strongly recommend that you call your insurer and verify your coverage prior to your first visit. All insurance plans are different. Please do not assume that your insurance will cover your therapy. Be sure to ask if your insurer offers different deductibles or different coverage for out-of-network providers. However, if you have a Health Savings Account (HAS), these funds can usually be used for physical therapy services. Contact me if you have any further questions. Treatment sessions are held one-on-one in a comfortable, clean, and private setting.
On the first visit (usually an hour long), Mary will perform a full evaluation including a pelvic examination. If it is determined that you have relevant pelvic floor dysfunction, your treatment will also be initiated on the first visit.
Treatments most often include instruction in self-treatment and exercises you will perform at home. The home program is integral to the healing process. Please wear comfortable clothing that allows for full range of motion to your treatment sessions.
Please bring the following to your first visit:
  • Photo ID (must be state or federally issued-Drivers License, State of Maryland Photo ID, Passport)
  • Insurance Cards - please remember to bring your primary insurance information
  • Any recent radiology films or reports
  • Medical records from any previous treating physicians or pain practice
  • List of current medications
  • Your referral for physical therapy treatment
